Medicaid Transportation

This reimbursable program allows for participants to receive transportation to and from medical appointments that are eligible and/or currently on Medical Assistance (MA), Supplemental Security Income (SSI), Badgercare or Medicaid. Access to volunteer drivers is subject to availability.

Services offered:

Reimbursement for traveling to and from medical appointments
Lodging and meals reimbursable when traveling out of town as necessary
Transportation provided by a volunteer driver or a driver that you have chosen and is approved by the Specialist

Criteria:

Eligible or currently on Medical Assistance (MA), Supplemental Security Income (SSI), Badgercare or Medicaid
Pre-authorization is required for each appointment
Any emergency must be documented within 72 hours by contacting Deb or Marion by telephone or leaving message on voicemail (779-3706)
