POSITION:  Pharmacist

 

LOCATION:  Red Cliff Community Health Center

 

SALARY:  Negotiable depending upon qualifications

 

THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION

 

SUPERVISOR(S):  Chief Pharmacist and Health Center Administrator

 

JOB SUMMARY:  The major responsibilities of the pharmacist are to contribute to restoring and maintaining the health of the service population by promoting the rational inclusion of pharmaceutical products within community health and patient care management plans; the correct dispensing, use and administration of pharmaceutical products by patients; clinical staff consultation and monitoring.  Additionally, the pharmacist ensures pharmaceutically and related products will be obtained from appropriate sources to assure quality, therapeutic efficiency, and reasonable costs.  The pharmacist will ensure that deferral mandated sources (PHS, SSC, VA, and FSS) are used.  All applicable federal and state laws, rules, and regulations will be compiled with in the purchase, handling, and distribution of pharmaceutical and related products and medical supplies.

 

DUTIES AND RESPONSIBILITIES: 

A.     Prescription Processing and Dispensing

  1. Health Center prescriptions will be written into the patient’s health record or through the Electronic Health Record.
  2. Prescriptions will be processed and dispensed in compliance with applicable federal and state laws, rules, and regulations.
  3. Pharmaceutical products will conform to current standards of quality for potency, purity and efficacy.  (USP and NF)
  4. Prescription containers will conform to requirements for protecting the integrity of pharmaceutical products and of the Poison Prevention packaging Act, unless other containers are more appropriate for meeting the needs of an individual patient.
  5. Prescription labeling will provide an identifying number, date dispensed, patients’ name, name or initials of the pharmacist, the name, address, applicable certification number, and telephone number of the Health Center pharmacy; required cautionary and supplementary labels, days of treatment and the name, strength and quantity of the pharmaceutical products, and instructions for use.
  6. Inventory CII narcotics and handle in the approved manner.
  7. Ensure that IHS funds, including grant and contract funds, will not be used in payment for drugs classified as ineffective and/or possibly effective for every indication by the Food and Drug Administration.
  8. In the event the pharmacy receives a prescription for drug classified as ineffective, the pharmacist will contact the prescribing physician to change the prescription to an effective drug.
  9. Dispense generic pharmaceutical products when authorized for patients with chronic disease on long-term drug therapy.
  10. Ensure that any generic drugs stocked are of acceptable quality according to nationally recognized standards.

 

B.     Clinical pharmacy and Counseling:

1.      Review the patient’s health records and medication profile to assure appropriate drug therapy and to reduce the potential for complications and known adverse reactions.

  1. Provide the prescribers suggested alternative drug therapy plans when any of the above risks are identified.
  2. Individually counsel patients on the purpose and use of the prescribed medications, potential side effects, adverse reactions, and other related factors.
  3. Enter appropriate patient and prescription information into the patient’s health record in a problem-oriented format and update the medication profile.
  4. Initial the medical record chard after the above items have been completed.

 

C.     Prescription Refills:

  1. The prescriber will authorize prescription refills prior to dispensing by the pharmacist.
  2. Refill quantities for patients with chronic diseases on long-term therapy, for not less than thirty-day supply unless the prescriber specifically directs otherwise.
  3. Collect the appropriate information to decide patient compliance with prescription therapy and in the event of adverse effects or therapeutic failure, he or she will contact the prescriber and medical director for guidance.

 

 

D. Supervision of Assigned Staff:

  1. Oversee the activities delegated to and/or assigned to the Pharmacy Technician.  These will include:

a.       Maintain documented evidence of patients’ insurance and other benefits; maintain the required records on self pay patients.

b.      The order and stocking of medications and supplies.

c.       The proper disposal of discarded documents to safeguard patient confidentiality.

d.      Checking out patient records for the pharmacist in accord with the established practice of the Medical Records Department.

 

 

E.      Standard Compliance:

  1. In conjunction with Health Center staff, evaluate pharmacy services in accord with plans, policies, and procedures, federal and state laws, rules and regulations and other applicable criteria.  The following sources of standards shall be used in the development of plans, policies and procedures:

a.       Indian Health Service Manual, Part 3, Professional Services, Chapter 7, Pharmacy, revised 2/7/82.

b.      Joint Commission on Accreditation of Hospitals, Accreditation Manual for Ambulatory Health Care, Chapter on Pharmaceutical Services.

c.       American Society of Hospital pharmacists Statement on the Provision of pharmaceutical Services in Ambulatory Care Settings.

d.      American Society of Hospital Pharmacists Minimum Standards for Ambulatory Care Pharmaceutical Services.

e.       Bemidji Area Pharmacy Quality Assurance Review Document.

 

F.      Other duties:

  1. Maintains strict confidentiality.
  2. Responsible for operations within the department to ensure that services are provided in a manner which meets all applicable regulatory licensure and accrediting regulations.
  3. Responsible for positive representation of facility and department services.
  4. Exemplifies personal conduct style which is reflected in an appropriate role.
  5. Upholds established professional standards.
  6. Maintains a cooperative relationship with employees, medical staff, and others
  7. Implement Quality Assurance measures in appropriate areas.
  8. Adheres to and supports facility policies, programs and activities.
  9. Responsible for communication within the Pharmacy department and with other Health Center departments to ensure effective operations.
  10. Present a professional, caring image for the Health Center and its programs.

a.       Maintain a cooperative relationship with other Health Center staff and co-workers.

b.      Exceptional Customer service is a must. Demonstrate tact, courtesy, and respect in communication and interaction with health Center patients, visitors, and staff and with outside agencies and programs.

c.       Promote a working environment noted for effective cooperation and collaboration between programs, services, and co-workers.

  1. Responsible for personal development to ensure current knowledge in the profession.
  2. Participate in the implementation of grants, contracts, or projects being carried out under the auspices of the Red Cliff Community Health Center
  3. Perform any other job-related duties as specified by supervisor.
  4. Attend meetings and in-services and any other events as specified by immediate supervisor.
  5. Responsible for maintaining a clean and safe environment.

                                

SUPERVISORY AUTHORITY: Pharmacy staff when Chief Pharmacist is not present.            

 

KNOWLEDGE:  Exposure to Health Hazards of the Health Industry.  Knowledge of the enforcement of safety and biohazard regulations.

 

QUALIFICATIONS:  Possession of a Bachelors Degree in Pharmacy or PharmD.  Must be registered as a Pharmacist with the State of Wisconsin, Department of Regulation and Licensing.  Possess a broad professional knowledge in the field of pharmacy, and aspects of the total pharmacy operations. Good interpersonal skills; the ability to get along well with diverse personalities. Good communication skills, both written and verbal.

 

PERSONAL CONTACTS: Daily contact with patients, visitors, primary care providers, Tribal and Health Center Administration, and other Health Center staff.

 

PHYSICAL REQUIREMENTS:  The duties assigned to this position involve bending, stooping, lifting, and carrying.  Items may be placed on overhead shelving.  Weights to be carried are usually less than 50 pounds.  There will be periods of prolonged standing, walking, or sitting.

 

WORK ENVIRONMENT:  Red Cliff Community Health Center; office and clinic settings. Exposure to hazards of the health care industry. Work setting must be maintained as a clean, non smoking, well-ventilated area in compliance with all applicable safety regulations.

 

 

TRAVEL REQUIREMENTS:  Possession of valid drivers license and having regular access to a motor vehicle with appropriate insurance coverage.  The ability to travel and attend meetings or trainings.  The availability for out of town and overnight travel. May be asked to perform visits to patient homes. May be required to travel to deliver or pick up x-rays, specimens, or Health Center/Clinic supplies.
