Program Description:
The Coordinated Services Team program is funded by a state grant. The program is to develop a team of service providers and community members to develop a program in which services for families may be coordinated between several service providers.

Families in need of services may be self referred or may be referred by community members or service providers. With the assistance of a case manager, families decide what they need to achieve balance and who can help them provide it.
