The Red Cliff Community Health Center offers a variety of outreach services for our community that have been specialized into Maternal Child Health (MCH) and Adult Health services.

Honor Our Children Project (HOC)
The HOC Project is a Maternal Child health Project that is focused on health and social services aimed at reducing infant mortality and morbidity rates in the Native American community. The program accomplishes these goals through advocacy, prenatal care coordination, incentives, breastfeeding education and support, childbirth education classes, smoking cessation counseling, transportation and home visitation.

Community Health Nurse (CHN) and Community Health Representatives (CHR)
The Community health team works collaboratively to provide a wide variety of health related services, with an emphasis on wellness.

Some of the services provided include; case management, immunizations, injury prevention, WIC, Optical program, diabetes support groups, employee health and in-services, community education, screening clinics, medication delivery, transportation, home visitation, and community wellness events.
